  • All sales on services and products are final.
  • There are no refunds or exchanges on products or gift cards.
  • Packaged pricing discount based on full pre-payment and is non-refundable.
  • Quote expires 90 days after print date unless otherwise specified on the quote.
  • Package Payment and Treatment Plan Terms
    • The total package price applies to a specific, time-limited treatment plan. All payments are earned upon receipt as non-refundable administrative and reservation fees used to secure clinical resources, professional staff time, provider availability, treatment planning, supplies, equipment, and clinical space.
      • A fee of $1,000 is a non-refundable fee, earned upon receipt, for medical history review, treatment planning, and treatment mapping.
      • The remaining Treatment Execution Balance is earned upon scheduling and reservation of clinical space, clinical equipment, required products and supplies, and provider hours. This payment is not a deposit and is non-refundable if the patient cancels, fails to schedule, or no longer wishes to continue with the treatment plan.
  • Service Window and Treatment Execution
    • To comply with clinical protocols, all treatment plans must be initiated and completed within 12 months from the date of purchase.
    • Upon expiration of the 12-month period, the treatment plan is considered fully executed for purposes of reserving clinical space, provider time, staff availability, equipment, and treatment resources. Any funds paid are not considered a credit balance, stored value account, or transferable balance. The package is non-transferable and non-refundable.
  • Surgical Patients 
    • A $1,000 non-refundable retainer fee, earned upon receipt, is due to secure the operating room and Dr. Bachilo’s time for the scheduled surgery date. This payment is not a deposit and is non-refundable if the patient cancels surgery. This fee secures our availability and covers the initial administrative work performed.
    • Full payment is required 2 weeks prior to the surgery date or at the time of the pre-operative visit, whichever comes first.
    • Should your plans change and you wish to cancel your scheduled procedure, we require a minimum of 30 (thirty) days’ notice prior to the surgery date in order to issue a full refund, minus the $1,000 non-refundable retainer fee.
    • If you cancel 15 (fifteen) days prior to the surgery date or after, you will be refunded 50% of the total procedure payment, minus the $1,000 non-refundable retainer fee. The remaining 50% of the total cost is retained as a non-refundable scheduling fee, while any amount paid above that will be returned.
    • All cancellations made within 14 (fourteen) days of the scheduled procedure are 100% non-refundable. Within this period, 100% of payment is earned upon receipt for medical history review, surgical planning, treatment mapping, coordination of care, reservation of the operating room, clinical resources, and provider/staff time.
  • Performance of any purchased services is at the discretion of our clinical staff. If you’re not a candidate for a service pre- purchased prior to a consultation, you may be eligible to apply the credit towards another service in our clinic.
